Early Intervention Process

This is a step process set by the New Mexico-Family Infant Toddler (NM-FIT) Program

Child Identification
You or another agency/individual have a concern about the development of your child (under the age of three and living in New Mexico).

Referral & Intake
Referral is made to a FIT provider agency.

A Service Coordinator from the local FIT provider will meet with you to arrange for a multidisciplinary evaluation. This evaluation occurs at your home or in a community location where your child is most comfortable. In preparation for the evaluation, you will be asked for information on your child's history. With your permission, the Service Coordinator will get copies of your child's medical records.

Evaluation
A Comprehensive Multidisciplinary Evaluation occurs. An evaluation report is written summarizing findings and making recommendations regarding your child's eligibility and, if applicable, provides approaches and strategies to be considered for service outcomes.


Eligibility Determination

Child is found NOT ELIGIBLE for FIT Program services

  • Your family is provided with information on growth and development
  • Referrals to other community resources, if appropriate, are made
  • Your child may be re-evaluated at a later date


Child is found ELIGIBLE for FIT Program Services

  • Individualized Family Service Plan (IFSP)
    At a setting and time convenient for you, an initial IFSP will be developed. You will work with a team of professionals to determine outcomes for your child and family and the services necessary to meet those outcomes.
  • Services
    Services are delivered according to the IFSP. Ongoing assessments are done; the IFSP is reviewed every six months to determine progress toward outcomes and the need for modification of outcomes or services.
  • Transition
    Prior to your child's third birthday, transition planning will occur to ensure his or her smooth effective transition from the FIT Program to other resources in the community, such as the public school's preschool special education services, Head Start or a daycare program. A transition conference will occur at least 90 days prior to your child's third birthday, where representatives from both the current EI provider and the transitioning agency are present.

Process takes approximately 45 days from the Referral & Intake to the IFSP.
